Regarding the issue you reported, I've just tried to fill in the form at my end and it's working as expected. The number of licenses I order does not make a difference either.1)
Which internet browser are you using? (Including version number)2)
Are you accessing the internet via a proxy server?
As a workaround, I would recommend that you simply forward the information that you've entered to sales [at] act-3d.com
(replace [at] with @), so that my colleagues can process your order manually.
Don't forget to attach the image/PDF file to the email.
